SHARED CARE PATHWAY:
THYROID & INTERVENTIONAL RADIOLOGY
1. OBJECTIVE
To establish a safe, efficient, and collaborative pathway for patients requiring specialized Thyroid Ablation or Surgical interventions, reducing waiting times while ensuring continuity of care between the UK Primary Care Provider (The Referrer) and Prof. Dr. Aykut Recep Aktaş’s Clinic in Antalya (The Provider).
2. RESPONSIBILITIES & REVENUE MODEL
A. Phase 1: Pre-Assessment (UK Side - Revenue for GP)
The UK GP retains full responsibility for the initial work-up. The patient remains under your care until travel.
- Required Actions:
- Current TSH, T3, T4, and Calcium blood tests.
- Neck Ultrasound (within the last 3 months).
- FNA Biopsy results (if available).
- "Fit to Fly" assessment (if co-morbidities exist).
Note: All pre-op consultations and tests are billed by the UK clinic directly to the patient.
B. Phase 2: Intervention (Antalya Side)
Prof. Dr. Aykut Recep Aktaş assumes full clinical responsibility for the procedure and immediate post-op care.
- Actions:
- Airport-Hotel-Clinic transfers.
- Detailed Pre-op Consultation & Consent.
- Procedure: Thyroid RFA / Microwave Ablation / Surgery / Prostate Fusion Biopsy.
- Post-procedure observation (24-48 hours depending on intervention).
- Discharge with English documentation.
C. Phase 3: Follow-Up & Monitoring (UK Side - Revenue for GP)
Upon return, the patient is directed back to the UK GP for monitoring.
- Required Actions:
- Wound Check: 7 days post-op (if surgical).
- Blood Work: TSH/Calcium levels check at Week 6.
- Ultrasound: Control scan at Month 3 to assess nodule shrinkage (for Ablation cases).
Note: Post-op follow-ups are billed by the UK clinic.
3. COMMUNICATION CHANNEL
- Direct Consultant Line: [Sizin WhatsApp/Acil Numaranız]
- Discharge Summary: Will be emailed to the referring GP's office within 24 hours of discharge.
4. LIABILITY STATEMENT
The Provider (Antalya) accepts full liability for the surgical/interventional procedure performed in Turkey. The Referrer (UK GP) is responsible only for the pre-travel assessment and post-return monitoring within their scope of practice.
